Blood Lead Reporting Solutions

Guidelines for reporting blood lead test results vary by state.  Contact your state’s Lead Poisoning Program for details.  For assistance, contact Product Support.

For states that utilize electronic reporting, LeadCare® II offers a complimentary software program for download.  States that currently accept the LeadCare II reporting software are listed in the dropdown below.  If your state requires electronic reporting and is not listed below, please contact the Director of your Lead Poisoning Program to inquire about compatibility.

State:

Arizona

Arizona has a new option for reporting that allows you to enter results directly into the AZ surveillance system. Please reach out to Healthy Homes to get a user account set up: healthyhomes@azdhs.gov.

Alternatively, Arizona requests that you use the Excel template below for reporting all of your lead results. Please click on the link below and save the file to your computer. Make note of the instructions on the first sheet and email your results to: healthyhomes@azdhs.gov.

Colorado

In cooperation with your state’s Lead Poisoning Prevention Program, we have created LeadCare Report software to streamline the reporting process.

Hawaii

Hawaii uses the National Electronic Disease Surveillance System (NEDSS) program to report lead results.
 
Alternatively, Hawaii requests that you use the Excel template below for reporting all of your lead results. Please click on the link below and save the file to your computer. Contact Kelly Hoffman at Hawaii DOH to set up and receive the password to use the state specified excel file. Make note of the instructions on the first sheet and email your results to: kelly.hoffman@doh.hawaii.gov.

Massachusetts

In cooperation with your state’s Lead Poisoning Prevention Program, we have created LeadCare Blood Lead Reporting Sofware Application v1.3.3 to streamline the reporting process.

New Jersey

North Carolina

Please contact Kim Gaetz at the North Carolina Department of Health and Human Services to set up reporting of blood lead results: kim.gaetz@dhhs.nc.gov or 919-707-5953.

To send LeadCare results through EMR, medical groups need to register through the NC DPH Meaningful Use site: https://ncdphmeaningfuluse.org.

New clinics can go to http://nchealthyhomes.com/webinars/ to view step-by-step training webinars on blood lead reporting in North Carolina, and how to use and submit LeadCare Reports. If you complete the evaluation at the end, you will receive a certificate for free CEUs.

Oregon

Oregon State currently requests that you submit electronically with ORPHEUS through an ELR or fax form from the website to (971) 673-0457.

An Excel template is also available below for reporting. Please click on the link below and save the file to your computer and send lead results securely via the State of OREGON secure ORPHEUS portal.

Pennsylvania

Pennsylvania Code requires that all blood lead test results be reported based on the following criteria:

Labs performing greater than or equal to 100 blood lead tests per month are required to report their results utilizing PA-NEDSS (Pennsylvania's version of the National Electronic Disease Surveillance System).

Labs performing less than 100 blood lead tests per month are required to report using the PA Childhood Lead Report Form.  Instructions for use are located in the footer of the document.

Rhode Island

Rhode Island currently requests that you use the Excel template below for reporting all of your lead results. Please click on the link below and save the file to your computer. West Virginia

The program is compatible with Windows 7, 8, and 10. 

Install the program and create a TEST Record Set to forward to your local Lead Surveillance Team after consulting with Product Support. Naming convention for the test Record Set: TEST_facility name w state_(date of creation) mm.dd.yyyy. Example: Test_Magellan_MA_12.01.2016
